Prayer Quilt Request Form

Please read before proceeding:

Prayer Quilts are available for family and friends of Siler members or the community.  They are typically requested in times of health and family crisis. However, they are not available for bereavement.

The requestor should ask the recipient if they are willing to receive a quilt with the prayers of the Siler family.  

There is no charge for the quilt. 

After receiving the request, a member of the Quilt Ministry team will contact the requestor to coordinate a date for the quilt's presentation.   

The requestor will arrange the delivery of the quilt.
